Fees and Payment

Fees must be paid in US currency.

Initial Examination Fee (includes nonrefundable $680 application fee) $1,700
Reexamination Fee $1,020
Extended Deadline Fee (applied after application deadline)  $500

Fees are paid online at the time of application. UCNS accepts VISA, MasterCard, and American Express for payment.

Certification Application Letter Templates and Special Testing Accommodations Form

Verification letters are required as part of each subspecialty’s online certification application. Applicants must use the verification letter templates. Letters submitted that do not follow the provided templates will not be accepted.

Applicants requesting special testing accommodations must submit the application for testing accommodations and supporting documentation substantiating the disability at the time of application. More information on testing accommodations can be found in the Certification Policies .
