Program Changes

UCNS must be notified in advance of any major program changes. To submit a change request, use the provided templates as a guide and submit the attachments indicated below. All signatures and attachments must be received before a program change request may be processed. Program change reviews are generally completed by the Accreditation Council within six to eight weeks once all materials are received. Failure to notify UCNS of changes as they occur may result in loss of accreditation by administrative withdrawal.

For a change in Program Director, please complete and submit:
  • Program Change Notification Letter (signed by the department chair)
  • Program Director CV
  • Program Director Information Table
For a change in the Sponsoring Institution, please complete and submit:
  • Program Change Notification Letter
  • Sponsoring Institution Letter (signed by ACGME-approved designated institution official)
  • Institution Tables
For a loss of significant resources, please complete and submit:
  • Program Change Notification Letter
For a discontinuation or addition of rotations to participating sites, please complete and submit:

  • Participating Institution Letter (for addition of a site)
For a change in program duration, including implementation of flexible fellowships, please complete and submit:
