Program Changes
UCNS must be notified in advance of any major program changes. To submit a change request, use the templates provided in the Accreditation Portal as a guide and submit the attachments indicated below. All signatures and attachments must be received before a program change request may be processed. Program change reviews are generally completed by the Accreditation Council within six to eight weeks once all materials are received. Failure to notify UCNS of changes as they occur may result in loss of accreditation by administrative withdrawal.
Once you have requested a program change, UCNS staff will create the form for you to submit through your program’s Zengine accreditation portal. Instructions to complete the program change may be found here. You will receive confirmation that your form has been created and is available for submission within five business days.

For a
change in Program Director:
- Program Change Notification Letter (signed by the department chair)
- Program Director CV (using required template)

For a
change in the Sponsoring Institution:
- Program Change Notification Letter (signed by the Designated Institution Official)
- Sponsoring Institution Letter (signed by ACGME-approved designated institution official)

For a
change in the Primary Institution:

- Primary Institution Letter (using required template signed by the Department Chair)

For a

For a
change in in program duration, including implementation of flexible fellowships and one-year tracks for Neurocritical Care:
