The application cycle for the Headache Medicine examination opens on January 1, 2014. The Headache Medicine certification examination will be offered the week of October 6-10. The exam is scheduled for four hours and will have 200 multiple-choice questions that are computer-based. Applications, supporting documents, and payment must be submitted by the application deadline of April 15, 2014.
The practice track pathway of application will close on the last day of the application cycle conducted for the 2014 Headache Medicine examination. After the 2014 application cycle only those physicians who have completed a UCNS-accredited fellowship program will be able to apply.
PLEASE NOTE: UCNS utilizes an online application system. All applications must be completed online, and all supporting documentation must be uploaded through the online application system at the time of application. Some application sections require you to upload documents before you will be allowed to proceed to the next section.
A personal logon is required to access the application. You must request your personal logon information no later than 4:00 p.m. Central on April 15, 2014 in order to apply for this examination. You are encouraged to work on your application prior to 4:00 p.m. Central on April 15, 2014 as application support will not be available and logon requests will not be granted after 4:00 p.m.
In order to have your application processed you must finalize your completed application by 11:59 p.m. Central on April 15, 2014. Any application not finalized by this time will not be accepted. In addition to a finalized application, UCNS must receive the $1,600 examination fee postmarked on or before April 15, 2014 if paying by check or money order. Payment is the only application item that may be mailed to the UCNS office. UCNS also accepts Visa, MasterCard, Discover, and American Express.
Applicants who have taken the exam in a previous year but were not successful and wish to reapply must submit a new application, provide current supporting documentation, and pay the current reexamination fee. Documentation submitted with a previous application will not be accepted.
You must use the template letters that are provided above. Any required letters of support are not meant to be letters of recommendation. They are intended to provide the necessary information to us in the most clear and concise manner possible. Letters that are confusing or do not provide the necessary information will not be accepted.
