Special Testing Accommodations Form

The Headache Medicine exam will be offered the week of October 1–5, 2012. The exam is scheduled for four hours and will have 200 multiple-choice questions that are computer-based

The practice track pathways will close following the 2012 examination. After 2012, only those who have completed a UCNS–accredited HM fellowship program will be eligible to apply for certification examinations in HM.
