The application cycle for the 2016 Autonomic Disorders certification examination opens on April 1, 2016. The Autonomic Disorders certification examination will be offered the week of December 5-9, 2016. The exam is scheduled for four hours and will have 200 computer-based multiple-choice questions. Applications, supporting documents, and payment must be submitted by the application deadline of July 15, 2016.
PLEASE NOTE: UCNS utilizes an online application system. All applications must be completed online, and all supporting documentation must be uploaded through the online application system at the time of application. Some application sections require you to upload documents before you will be allowed to proceed to the next section.
A personal logon is required to access the application. You must request your personal logon information no later than 4:00 p.m. Central on July 15, 2016, in order to apply for this examination. You are encouraged to work on your application prior to 4:00 p.m. Central on July 15, 2016, as application support will not be available and logon requests will not be granted after 4:00 p.m. In order to have your application processed you must finalize your completed application by 11:59 p.m. Central on July 15, 2016. Any application not finalized by this time will not be accepted. In addition to a finalized application, UCNS must receive the $1,700 certification examination fee postmarked on or before July 15, 2016, if paying by check or money order. Payment is the only application item that may be mailed to the UCNS office. UCNS also accepts Visa, MasterCard, and American Express.
Applicants who have taken the exam in a previous year but were not successful and wish to reapply must submit a new application, provide current supporting documentation, and pay the current reexamination fee. Documentation submitted with a previous application will not be accepted.

They are intended to provide the necessary information to us in the most clear and concise manner possible. Letters that are incomplete or do not follow the provided templates will not be accepted.
